<h1>What Actually Breaks First in a High-Performance Operating System?</h1>

<p>The previous article in this series mapped the formal architecture of a high-performance operating system <a href="#ref-11" class="citation">[11]</a>. The layers. The decision rights. The accountability structures that are supposed to hold the system together under pressure.</p>
<p>This article asks a different question.</p>
<p>Not what the architecture looks like. What it does under load. And specifically: when a high-performance system fails, what actually breaks first?</p>
<p>The standard answer points to the visible event. A scandal surfaces. An athlete comes forward. A parliamentary committee convenes. An independent review is commissioned. The board is cleared out. New leadership is appointed. New policies are written. The narrative resets.</p>
<p>That sequence is accurate about the order of visible events. It is largely wrong about the order of actual failures.</p>
<h2>The Visible Event Is Not the First Failure</h2>
<p>James Reason's work on organisational accidents, particularly his Swiss Cheese Model of system failure <a href="#ref-1" class="citation">[1]</a>, has underpinned how aviation, healthcare, and nuclear industries understand failure for several decades. He drew a distinction that sport governance has been slow to apply.</p>
<p>Active failures are the visible events. The decision that triggers the crisis. The individual whose action breaks into public view. These are the things that appear in headlines.</p>
<p>Latent conditions are the structural preconditions. The unchecked authority. The information that was never escalated. The cultural norm that routinely overrode formal process. The governance gap that nobody named because naming it would require someone to own it. These accumulate over time, often invisibly, until an active failure triggers a crisis that exposes them all at once.</p>
<p>The critical point is this. Across the safety science literature <a href="#ref-1" class="citation">[1]</a><a href="#ref-2" class="citation">[2]</a>, latent conditions are consistently identified as the primary cause of complex system failures, with the active failure as the trigger that exposes them. Treating the trigger as the cause produces responses that address the wrong problem.</p>
<p>This framework was developed in safety-critical industries and applies most precisely in those contexts. Sport governance is not identical to aviation or healthcare. Volunteer boards, the pressures of the Olympic cycle, and the political economy of national sport funding create conditions that do not have direct equivalents elsewhere. The analogy has limits and those limits need to be named.</p>
<p>What the framework does is give practitioners a vocabulary for something they have observed but have not had precise language for. The visible event is rarely the first failure. It is the point at which accumulated failures become impossible to contain. Two documented cases make this concrete.</p>
<h2>Case One: Russian State Doping</h2>
<p>The McLaren Report, commissioned by WADA and published in two parts in July and December 2016 <a href="#ref-3" class="citation">[3]</a><a href="#ref-4" class="citation">[4]</a>, concluded, beyond reasonable doubt, that Russia's Ministry of Sport had directed a state-operated system for subverting anti-doping processes across multiple Olympic cycles. The programme implicated more than 1,000 athletes across summer, winter, and Paralympic sports. Sample swapping. Destroyed urine records. State security service involvement in protecting positive test results.</p>
<p>The visible crisis arrived in 2016. The failure sequence started well before it.</p>
<p>In July 2013, journalists Nick Harris and Marthe Kelner published a report in the Mail on Sunday documenting state-sponsored doping in Russia, centred on the WADA-accredited Moscow laboratory. Harris has documented, in a first-person account published in Sporting Intelligence in July 2016 <a href="#ref-5" class="citation">[5]</a>, that he contacted WADA on 26 June and 2 July 2013, and the IOC on 2 July 2013, with specific allegations about the laboratory director and the doping programme.</p>
<p>WADA responded that it could not comment on specific allegations and had no investigative powers under the existing Code <a href="#ref-5" class="citation">[5]</a>. The IOC acknowledged receipt on 3 July, promised on 5 July to follow up, and never did. According to an academic chapter published by Play the Game <a href="#ref-6" class="citation">[6]</a>, Harris and Kelner also informed the IAAF, RUSADA, and the Russian Ministry of Sport, and received no substantive response from any of them.</p>
<p>Seventeen months elapsed between the Mail on Sunday publication and WADA confirming, in December 2014, that it would investigate. That confirmation came only after a German ARD documentary broadcast further detailed allegations. WADA's own chief investigator later indicated that media attention had been necessary to force the issue <a href="#ref-6" class="citation">[6]</a>.</p>
<p>In November 2015, the WADA Independent Commission <a href="#ref-7" class="citation">[7]</a> found the laboratory director was at the heart of a positive test cover-up and had directed the destruction of 1,417 urine samples ahead of an audit. The McLaren investigation itself was not commissioned until May 2016, approximately three years after Harris and Kelner's first documented contacts with WADA and the IOC.</p>
<p>The programme was operational from at least 2011 <a href="#ref-4" class="citation">[4]</a>. The gap between the first documented signals to the governance bodies and a formal investigation response is roughly three years. The gap between programme operation and formal response is approximately five.</p>
<p>What broke first was not the anti-doping testing mechanism. The testing mechanism was being deliberately circumvented. What broke first was the escalation architecture. The structure by which early signals should have produced institutional action did not function. Signals arrived. They were not carried to the level of authority required to act on them. The latent condition was a governance structure that received information and did not convert it into response. The active failure was the moment continued inaction became publicly untenable.</p>
<p>The 2016 crisis looked like an anti-doping failure. Structurally, it was a governance failure that had been accumulating for years before anyone named it.</p>
<h2>Case Two: Canada's Future of Sport Commission</h2>
<p>Canada's Future of Sport Commission, which published its final report in March 2026 <a href="#ref-8" class="citation">[8]</a>, produced one of the most detailed accounts of systemic failure in national high-performance sport available in any jurisdiction. The commission arrived at 98 calls to action <a href="#ref-9" class="citation">[9]</a> across governance, athlete welfare, accountability, and funding architecture.</p>
<p>That number deserves a moment of attention. Ninety-eight calls to action are not a reform agenda. They are a measurement of what was allowed to accumulate.</p>
<p>The commission's findings were not primarily about individual misconduct. They were about structural conditions that had developed over many years without being named or corrected. The commission documented a sport system that is, in its own words, underfunded and fragmented, lacking clear national leadership, with governance frameworks that are inconsistent, paper-based, and regularly not operationalised in practice <a href="#ref-8" class="citation">[8]</a><a href="#ref-9" class="citation">[9]</a>. The commission's preliminary report documented that sport organisations were governed by volunteer-led boards whose members frequently lacked governance experience or expertise, yet were expected to oversee organisations managing significant budgets and increasingly complex operations <a href="#ref-8" class="citation">[8]</a>.</p>
<p>The commission identified a pattern that is directly relevant to the failure sequence argument. Maltreatment in sport was found to be widespread, systemic, and ongoing, with winning, reputation, and funding too often prioritised over participant safety and well-being <a href="#ref-9" class="citation">[9]</a>. High-performing programmes were accumulating welfare and governance failures beneath strong competitive results. Performance outcomes were suppressing the escalation mechanisms that would otherwise have surfaced problems to governance boards. The system was producing results. The system was also accumulating conditions it had not yet been required to account for.</p>
<p>The structural explanation is specific. When performance outcomes are strong, the informal authority of operational leaders expands. Scrutiny is withheld. Concerns raised internally are read against the backdrop of winning. The question asked of a complaint is not "is this structurally significant?" but "is this worth disrupting something that is working?" That question, asked consistently over time, produces an organisation that has learned not to escalate.</p>
<p>By the time the visible failures became impossible to ignore, the conditions behind them had been accumulating across cycles. The commission did not find a series of isolated breakdowns. Commissioner Lise Maisonneuve described a system requiring decisive and sustained action to restore trust <a href="#ref-9" class="citation">[9]</a>, with long-standing governance failures and inadequate oversight having allowed maltreatment to continue across the national sport system.</p>
<p>What broke first, in the structural reading of the cases the commission examined, was the information architecture: the formal mechanisms by which operational reality reaches governance oversight. The mechanisms were not functioning as designed. Not because they were absent from the governance documents. Because the cultural and structural conditions that had accumulated made using them effectively impossible <a href="#ref-10" class="citation">[10]</a>.</p>
<h2>The Failure Sequence</h2>
<p>Across these two cases, and consistent with the safety science literature already cited, a pattern is visible.</p>
<p>A constraint must be stated before going further. Inquiry reports are retrospective. They identify what went wrong after it has become publicly visible. They do not map latent failure accumulation in real time and they do not do so with the analytical precision of an aviation safety investigation. The sequence described below is a framework for structured thinking, not a proven causal model for sport governance specifically. Apply it as a lens, not a blueprint.</p>
<div class="numbered"><span class="num">1.</span> <span class="label">Information failures come first.</span> Early signals exist. Complaints are made. Anomalies are observed. Informal concerns circulate at the operational level. The escalation architecture does not carry them to the governance layer. Sometimes this is active suppression. More commonly it is structural. The formal reporting pathway terminates below the level of authority required to act. Nobody decides not to escalate. The system simply does not make escalation the path of least resistance.</div>
<div class="numbered"><span class="num">2.</span> <span class="label">Cultural conditions form second.</span> Informal norms develop that override formal processes. Results suppress scrutiny. Relationships create authority structures that sit alongside the documented ones. People who raise concerns learn, through experience, that the personal cost of doing so is real and that the institutional response is unreliable. This is not a cause of failure. It is what happens in the extended period after the information architecture has already broken down and nobody has repaired it.</div>
<div class="numbered"><span class="num">3.</span> <span class="label">Governance authority drifts third.</span> The gap between documented decision authority and operational decision reality widens. Boards govern the system on paper. Operational decisions are made in structures the board does not control and, in some cases, does not know exist. The formal governance architecture becomes a description of how the system is supposed to work rather than how it works. This condition is very rarely visible from inside the governance layer without deliberate diagnostic effort.</div>
<div class="numbered"><span class="num">4.</span> <span class="label">The visible failure arrives last.</span> By the time the crisis surfaces, it has been preceded by failures in information flow, cultural normalisation, and authority drift. The visible event is real. The individuals whose actions triggered it are responsible for those actions. But the visible event is not the primary cause of what went wrong. It is the trigger that makes accumulated conditions publicly impossible to ignore.</div>
<h2>On Individual and System Responsibility</h2>
<p>This needs to be stated plainly, because the argument is sometimes misread.</p>
<p>Identifying structural conditions as primary causes of governance failure does not mean that individuals bear no responsibility for their choices within those conditions. Both things are true simultaneously. A system that creates the conditions for failure does not absolve the people who made specific decisions within it. Accountability for specific actions and structural analysis of the conditions that enabled those actions are not in competition.</p>
<p>What the structural argument changes is the framing for prevention.</p>
<p>If the primary response to a visible failure is to replace the individuals and rewrite the policies, without addressing the structural conditions that allowed the failure to develop and remain hidden, the same conditions will produce equivalent outcomes under different names. Sport governance has demonstrated this pattern repeatedly. New leadership. New codes. New accountability frameworks. The same structural conditions, re-inherited by a different set of people.</p>
<p>The question for a board chair or CEO is not only "who is responsible for this?" That question matters. It is not sufficient on its own.</p>
<p>The supplementary question is: what was structurally broken before this happened, and for how long?</p>
<p>Those are different questions. They require different responses.</p>
<h2>The Rule</h2>
<p>If you want to understand what will break first in your high-performance operating system, do not start with your operational performance metrics. Start with your information architecture.</p>
<p>Four specific places to look:</p>
<div class="numbered"><span class="num">1.</span> <span class="label">The escalation pathway.</span> When a problem is identified at the operational level, what is the formal route to the governance layer? How many steps does it pass through? At each step, what are the conditions under which it is filtered, delayed, or reframed before it arrives? A pathway that exists on paper and is not used in practice is not a functioning pathway.</div>
<div class="numbered"><span class="num">2.</span> <span class="label">The cultural override conditions.</span> Where in your system do informal norms routinely take precedence over formal process? Not where it is theoretically possible. Where it actually happens. The answer to this question is rarely in any document. It is in how people behave when there is no external scrutiny.</div>
<div class="numbered"><span class="num">3.</span> <span class="label">The authority gap.</span> Where does documented decision authority sit, and where are operational decisions actually made? The distance between those two points is not an organisational chart problem. It is a governance risk. The wider the gap, the longer the latent failure accumulation period before it becomes visible.</div>
<div class="numbered"><span class="num">4.</span> <span class="label">The board's information diet.</span> What does your board actually receive? Who decides what it receives? What are the structural conditions under which information that would trigger governance scrutiny does not arrive? A board that is only informed of what operational leadership chooses to surface is not governing. It is ratifying.</div>
<p>These are not comfortable questions. A board that asks them seriously will not always like the answers.</p>
<p>That is the point.</p>
<h2>What This Means Practically</h2>
<p>The standard governance posture is retrospective. Review results against plan. Assess performance data. Respond to problems that have already surfaced. This is necessary. It is not sufficient.</p>
<p>The cases above did not fail because their governance bodies were failing to do retrospective governance. They failed because the conditions that produced the visible crises were invisible to the governance layer until they could no longer be contained. The retrospective posture addressed what was already visible. The latent conditions developed in the space it did not reach.</p>
<p>The supplementary posture is prospective. Not "what went wrong?" but "what structural conditions exist in your system right now that, under sufficient pressure, would produce a failure you cannot currently see?"</p>
<p>That question is uncomfortable for a reason. It cannot be answered from a performance dashboard. It cannot be answered by reviewing what has already been escalated. It requires a different kind of tool: a structured stress-test that asks what the system does when its formal processes are bypassed, not followed, or simply not used.</p>
<p>If you do not have that diagnostic in place, the failure sequence is operating in your system right now. You just cannot see it yet.</p>
